One of the main reasons for linked accounts when you don't recognize the other name is through third party affiliation. Have you ever used a VA or other third party to help with your account?

Remember to remove any permissions, too, if they had permissions to your account.
Sounds like you've given them everything possible. If you still receive a rejection with all that, I don't really have anything else to offer.
